This is a sample that can be adjusted to conform to your school's requirements for enrolling in a consortial course through Acadeum.
In order to be considered eligible to enroll in consortial courses (CC) online through the [School name]/CIC partnership, students must:
- Be enrolled full-time (at least 12 credit hours) at [School Name] during the academic term in which they are seeking to take an Acadeum course (Acadeum courses will be included in determination of full-time status);
- Be in good academic standing. Students who are on academic probation are NOT eligible to enroll in Acadeum courses;
- Be at the sophomore level (at least 30 sch earned) at the time of enrollment in a Acadeum course;
- Be financially cleared at [University] in the academic term in which the student seeks to enroll in Acadeum courses.
All Acadeum enrollment action (add, drop, withdraw) requests must be made through the [insert title of enrollment coordinator], academic department chair or dean.
If your enrollment request is accepted by [School Name] and the teaching institution, please initial below to indicate that you understand and accept the following terms:
______ Each course requires approval from the Vice President of Academic Affairs, [insert title of enrollment coordinator], deans, and department chairs based on their review of each course.
______ Enrollment in online courses is permitted up to six (6) semester credit hours per academic term. These courses are labeled [label here] in place of course section numbers.
______ Upon completion of each Acadeum course, Office of the Registrar and Student Records will post student’s final grade on the student’s transcript. Previously posted final grades will remain on the transcript, however, the Acadeum grades will be counted in the GPA and the credits hours will also be counted in the “attempted” and “earned” categories for Satisfactory Academic Progress calculations.
______ Students are responsible for adhering to course participation and policies of the teaching institution, which includes logging in on the course start date, submitting assignments, securing textbooks and learning materials, and following through on all required actions as stated on the syllabus and by the teaching institution.
______ Textbooks for Acadeum courses are not included in the tuition and fees.
______ [School Name] will not intervene in grade disputes in cases where a student is dissatisfied with the final grade issued by the teaching institution.
